How you unlock it
Campaign Operators are earned by finishing the named mission. That is a one-time unlock — once the mission is complete, the Operator stays on the account.
Where it shows up
Activision states that Operators are available in both Multiplayer and DMZ, and that each loadout can be assigned its own Operator. That means the Operator choice is per class rather than per account, so you can run different Operators in different slots of the same match.
What is not published
Activision has not published what any Operator does in combat. Operators are characters, and the official launch guide covers only how they are obtained. Treat any published Operator ranking as untested.
