Three slots, six Perks each
Create-a-Class gives you one Perk per slot, and every slot has six launch Perks. That means eighteen combinations in total, and a great many more once you account for the choices you make inside each Perk.
Read the unlock curve, not just the list
The first two Perks in every slot land at player level 4, which is the same level Create-a-Class opens. The curve then spreads out — the last Perk in each slot arrives between levels 31 and 48. In practice that means you will commit to an early Perk long before the rest of the slot is available, so the first pick in a slot matters more than the later ones.
What is not published
Activision's launch equipment guide lists the Perk names, the slot each belongs to and the player level it unlocks at. It does not publish what each Perk does in Modern Warfare 4, and the names are shared with a long-running series whose behaviour has changed between entries. This page therefore does not describe effects — check them in the live game and treat any published tier list as untested.
